Asian Cultural/Patient Support Service
Our Vision: To make a healthy difference


WHO ARE WE

THE TEAM: comprises 2.5 full/part time equivalent Chinese and Korean cultural support workers with different professional qualifications, different backgrounds, and different experiences covering both North Shore and West Auckland. The team are bilingual and can speak Mandarin, Cantonese and Korean.


WHAT SERVICES DO WE OFFER

The Asian Cultural/Patient Support Service offers community and hospital based support to people who need information about NZ Health System, cultural / emotional / language support for Chinese and Korean non-English speaking clients.

Check if patients need any help/ have any issues
Provide communication tool
Promote Asian health services and Guide to NZ health system

One-off /brief patient support
emotional /cultural/basic non-medical communication support for patients & their families

Patient support up to 4 weeks with a review
Cultural, emotional and basic communication support for WDHB staffs
Navigate and support clients to understand the health system
Support clients at clinical meetings
Identify and refer clients to other community agencies when need arises (e.g. to NGO CSW, CADS, Shakti etc.)

Provide cultural advice if staff needs it

Chinese and Korean bilingual staffs support this line to provide information and advice on general health information and assist Chinese and Korean people with language difficulties booking a GP or accessing services or information.

Breast Screening Support
Promote, recruit, enrol and support 45-69yrs Asian women to access free Breastscreening Referral Criteria: Asian women who meet eligibility criteria and lives in WDHB region

Asian Smokefree Communities Service
Promote and refer Asian people who wish to quit smoking or wish to go smokefree (home, car, workplace) to Asian smokefree communities service for smoking cessation and smokefree interventions.

Regional CADS - Asian counselling service
Promote and refer to Asian people seeking help to deal with alcohol & drug problems.

Quick Guide to NZ Health System Guide book
Provide talks and materials in Chinese, Korean and English versions available for patients.

Diabetes / Heart related medicine information (translated booklet)Provide free booklet to people who need.

Provide short or long-term practical and personal assistance to clients who have health problems or disabilities that prevent them from caring for themselves properly (NASC referral required).

SERVICE HOURS

Information line : Monday to Friday 9.00am to 5.00pm (Monday to Friday)
Patient support / Cultural Support : 10am to 2:30pm (Monday to Friday)

(not available during Statutory Holidays and weekends)
