Asian Cultural/Patient Support Service


Who can use our service (Referral)

Eligibility Criteria & Referral Form

The Asian Cultural /Patient Support workers work with Chinese or Korean patients of WDHB’s North Shore and West Auckland hospital and community health services, who requires engagement, basic communication, cultural and emotional support and /or are not familiar with health services/system, or knowledge of how to access appropriate and  relevant agencies due to language issues.

The information in this form is used to access our service, if meets the following eligible criteria.

Criteria for patient support referral:
checkcheckPatients who reside in North Shore, Waitakere area
checkcheckPeople from Chinese and Korean backgrounds
checkcheckUnder the care of clinical team, GP or other WDHB health practitioners
checkcheckNon-Mental Health patient (MH patients can get support from Asian MH Team)

How to make a Chinese/Korean Patient Support Referral

Urgent referral = request require immediate response 
Non-urgent referral = request require following day or non-urgent response 

For urgent over the phone referrals, you will be asked the following information:

  • Your name, service/department, phone number
  • Client’s Name; Client’s NHI number; spoken language/ethnicity
  • Purpose of the referral
For online referrals, you will be required to complete the referral form with the minimum information of:
  • Your name, service/department, phone number
  • Client's Name; Client's NHI number; spoken language/ethnicity
  • Purpose of the referral
